- Collect and verify required intake documentation (demographics, insurance information, physician orders, authorizations, etc.)
- Coordinate initial assessments and communicate scheduling with clinical staff
- Verify insurance eligibility and benefits when applicable
- Enter client data accurately into EMR/EVV systems
- Ensure all intake documentation meets state, payer, and company compliance standards
- Maintain communication with referral sources, case managers, and families
- Track referral conversion metrics and maintain intake logs
- Follow up on pending documentation and authorizations to avoid delays in start of care
- Provide administrative support related to admissions and census trackingQualifications
- High school diploma required
- Previous experience in home health, healthcare administration, or medical intake preferred
- Knowledge of insurance verification and authorization processes
- Familiarity with EMR systems and electronic documentation
- Strong organizational and multitasking skills
- Excellent written and verbal communication skills
- Professional phone etiquette
- Ability to maintain confidentiality and handle sensitive information
