Job Description

Status: Full Time Hourly

FLSA Classification: Non-Exempt

Salary: $45,000

Summary of the Position: Intensive Family Coordinator provides a professional level of case management services as well as direct advocacy services to a caseload of families.

  • Provide professional case management and direct advocacy services to children, youth, and families, ensuring services are tailored to individual strengths, needs, and goals.
  • Conduct comprehensive family assessments, recruit and support foster parents, complete home studies for referring agencies, identify community resources, and develop individualized service plans that promote family stability and well-being.
  • Coordinate all aspects of case management, including intake assessments, safety planning, crisis intervention, team meetings, service plan development, ongoing case monitoring, and detailed documentation in compliance with state regulations and agency standards.
  • Provide responsive support to families, including participation in an on-call rotation and availability to address urgent concerns and critical family needs.
  • Collaborate with families, foster parents, community partners, schools, and service providers to ensure coordinated care and successful outcomes for children and families.

Qualifications/Requirements:

  • High School Diploma or GED is required.
  • Experience with community work and at least one (1) year experience working in human services field.
  • Experience in the provision of home and community-based services.
  • Proficient in Microsoft Office Suite cand computer skills. Experience using an electronic health record (EHR) system is a plus.
  • Strong communication and organizational skills
  • Reliable transportation, valid driver's license and auto insurance coverage is required.
  • Bilingual (Spanish Speaking) is a plus.
  • Flexible Schedule - May require evenings and weekends
