This role offers the opportunity to make a meaningful impact by providing comprehensive care to patients of all ages (or adults) in a close-knit, underserved community. Rural medicine allows the delivery of high-quality primary care, including preventive, acute, and chronic disease management, to participate in community health initiatives, and promote wellness education.
- High elderly population
- Experienced in Chronic Disease Management
- 18+ patients per day
- Monday through Friday 8-5
- 36 patient hours, 4hrs of admin time- NO CALL
- EMR is Meditech Expanse, Dragon dictation
- Supported by an RN Team leader and medical assistant
- Available Specialty service include: Integrated Behavioral Health team, Audiology, Cardiology, Dermatology, ENT/Allergy, General Surgery, Neurology, Orthopedics, Podiatry, Pulmonary Medicine, Radiology, Sleep Medicine, Tele-Medicine Rheumatology, Urology, Wound Center, and more.
- Board Certified / Board Eligible- Family Medicine Physician or Internal Medicine Physician
- Ability to hold a New Hampshire state license
