Job Description

Southwest Behavioral & Health Services is seeking a Lead Social Worker to join our Community Resilience Center! Our Lead Social Worker goal is to assists’ persons to be participants in their chosen communities in the fullest manner possible. The principles and practice of recovery are emphasized, so individuals are able to bring new meaning and an enhanced quality of life into their daily experience. Together, integration and recovery offer consumers greater opportunities to lead satisfying and productive lives.

Schedule/Location: this position works Monday-Friday, 8am to 4pm. SB&H Community Resilience Center is located off of 27th Ave and Campbell.

Pay: starting salary is $60,000 per year with benefits.

About the Position

As a Lead Social Worker with SB&H, you will be responsible for providing timely, evidence-based clinical and non-clinical support services to adults, including screening, assessment, treatment planning, care coordination , running groups, developing programming, case management and crisis intervention. The position will assist in developing and implementing therapeutic programming for members including group counseling, activities and outings.

Duties & Responsibilities

  • Individual counseling
  • Group counseling
  • Completing intakes for new member enrollment
  • Complete intakes for new CRC members & annual updates for existing CRC members
  • Monitor CRC Master List for clinical document compliance
  • Monitor CRC LAG List for members who are not programming w/regularity – outreach protocols
  • Review & Sign clinical documentation
  • Train CRC staff on proper completion of clinical documentation
  • Complete assessment attestations w/CRC staff so they can complete intakes/annual updates on their own
  • Providing supervisory support and clinical training to front line staff

Minimum Requirements

  • Minimum requirement consists of a 21-year age
  • Master’s Degree in Social Work.
  • Licensed at the Associate or Independent level by the Arizona Board of Behavioral health Examiners
  • Required to maintain a valid Arizona driver’s license and pass motor vehicle screening for insurance purposes in order to transport clients, based upon need/requirements of the clinic and/or program.
  • Must have fingerprint clearance issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire per program requirements.
