Licensed Clinical Social Worker

Duties/Responsibilities:

Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each memberPlan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriersNavigate managed care plans for community services and programsCase management for social and behavioral care to allow members to self-manage health and social service supportAccurate and timely documentation of patient encounters and sessions in all clinical management systemsProactively identify methods to improve ArchWell Health's approach based on feedback and regularly conducted surveysSupport advance care transitions with members and their familiesAssist members with access to state-based prescription programs and other benefitsCollaborate with the primary care teams to conduct behavioral health assessments to determine the appropriate level of care for each member; screen and access each member for common mental health and substance use disordersDevelop care plans and provide short-term counseling or facilitate referrals, including warm hand offs for telehealth visits or referrals to external specialistsAccurate and timely documentation of patient encounters and counseling sessions in all clinical management systemsProactively identify methods to improve ArchWell Health's approach based on feedback and regularly conducted surveys

Required Skills/Abilities:

Experience supporting patients with co-occurring behavioral health and substance abuse issuesExperience with screening, assessment, and treatment planning for common mental health and/or substance use disordersWorking knowledge of differential diagnosis of common mental health and/or substance use disordersHighly skilled in behavioral therapy, cognitive behavioral therapy, dialectical behavioral therapy, relapse prevention therapy, contingency management, and motivational interviewingStrong interpersonal communication skills with exceptional active listening abilitiesHighly empathetic, non-judgmental, and open-mindedExperience in a collaborative team environment

Education and Experience:

1 years' experience in clinical social workA problem-solving orientation and a flexible and positive attitudeMission driven and motivated to join an organization that will transform the way we deliver accessible, clinically excellent care to seniorsFully Licensed LCSWMaster's degree in social workA problem-solving orientation and a flexible and positive attitude
