Location: Castro Valley, CAEmployment Type: Full-Time (W2)Schedule: Monday–Friday | 8:00 AM – 5:00 PMCompensation: $110,000 – $125,000 annuallyBenefits: Full benefits package & health insurance
Position Summary
We are seeking a Licensed Clinical Social Worker (LCSW) to join our integrated primary care team. This role provides short-term, solution-focused therapy (typically 10–12 sessions) to adult patients with mild to moderate concerns, while partnering closely with medical providers to support whole-person care.
Key Responsibilities
- Conduct assessments and develop individualized treatment plans
- Provide evidence-based therapy (CBT, behavioral activation, problem-solving therapy)
- Screen for behavioral health needs, trauma history, and substance use concerns
- Facilitate psychoeducational groups and coordinate referrals when needed
- Maintain accurate, timely documentation in the EMR
- Participate in interdisciplinary meetings and care coordination activities
Requirements
- Master’s degree in Social Worker or related field
- Active California LCSW license (AMFT accepted; supervision provided)
- Minimum of one (1) year of clinical experience
- Working knowledge of DSM-5 and evidence-based approaches
- Basic proficiency in Microsoft Office and EMR systems (OCHIN Epic preferred)
Preferred Qualifications
- Integrated care or community health experience
- Trauma-informed care training
- Spanish fluency is a plus
