Always Compassionate Health is a Licensed Home Care Services Agency providing exceptional community-based nursing care across the New York metro area. We are seeking a Licensed Master of Social Work for evaluations, nursing guidance, and monthly chart reviews of ventilation patients we are currently servicing.
Qualifications/Education:• NY State LMSW license• 2 years working as a social worker in a clinical environment.• Experience working in Home Care as a Social Worker preferred.Responsibilities:• Assess client home environment, factors impacting provision or barriers to client’s care.• Make recommendations for community and other resources for clients and family which facilitate the established plan of care.• Participate in client conferences as required.• Must be able to lift 50lbs.• Must be able to stand/work for extended periods.• Additional responsibilities as required.Always Compassionate Health is committed to the principle of equal employment opportunity for all employees and to providing employees with a work environment free of discrimination and harassment.
