Responsibilities
Assist licensed staff with patient care tasks and daily living activities.
Observe and report changes in patient condition to the nursing team.
Ensure patient safety by following care plans and safety protocols at all times.
Maintain accurate documentation of care provided and patient status.
Assist with mobility, transfers, and activities of daily living as directed.
Contribute to a performance-driven environment by meeting individual care metrics.
Maintain a clean, organized work area and comply with infection control standards.
Support teamwork by communicating effectively with colleagues and supervisors.
Participate in ongoing training and professional development opportunities.
Assist with routine patient monitoring tasks to support skilled nursing staff.
Escalate concerns promptly and follow escalation protocols when needed.
Demonstrate a proactive approach to improving care quality and patient experience.
Experience/Qualifications
Licensed Nursing Assistant (CNA) certification required.
Provide patient care under supervision and assist with daily living activities.
Monitor patient condition and report changes promptly to nursing team.
Follow care plans and infection control protocols at all times.
Accurate documentation of care and patient status in charting systems.
Assist with mobility, transfers, and activities of daily living as directed.
Escalate concerns promptly following established protocols.
Maintain clean, organized work area and equipment; comply with safety standards.
Participate in ongoing training and professional development opportunities.
Demonstrate reliability in a results-driven, metrics-focused culture.
