Qualifications

Minimum 1 year of current LPN experience in an acute care setting (home care experience preferred)
Authorization to work in the United States
Valid NYS LPN license
Valid BLS certification
Valid driver’s license (preferred)
Physical exam within the past year
QuantiFERON within the past year or chest X-ray within the past five years
MMR immunizations within the past ten years
