Job Description

We’re not just a home health agency — we’re a community built on trust, dignity, and the belief that every patient deserves to thrive in the comfort of their own home.We are currently seeking dedicated Licensed Practical Nurses (LPNs) to provide one-to-one private duty nursing care in patients’ homes. This is an excellent opportunity for an LPN who values flexibility, professional growth, and making a meaningful difference in patients’ lives. We offer stable, consistent case assignments, and per diem positions are also available.
- Compensation:
- $37–45 per hour, depending on experience and case
- Differentials available for high-acuity cases
- Referral and Sign-up Bonuses Available

- Responsibilities:
- Provide skilled nursing care in accordance with physician orders
- Document patient condition and care provided
- Follow patient care plans
- Communicate with the clinical team
- Requirements:
- Active NYS LPN License
- CPR Certification
- Infection Control Certificate
- Ability to work the assigned schedule
- Home care or PDN experience preferred
- Strong communication skills in English (bilingual in Spanish or Russian is a plus)
