We are currently seeking Licensed Practical Nurses (LPNs) who are passionate about making a difference and growing their careers in a collaborative healthcare environment.
About the Role
The Licensed Practical Nurse (LPN) plays a vital role in delivering safe, efficient, and patient-centered care across Community Health Net locations. This position includes a $3,000 sign-on bonus, payable in accordance with company policy and contingent upon meeting all employment requirements.
LPNs may be required to float between CHN locations based on operational needs.
Key Responsibilities
- Conduct and accurately document patient health histories and assessments
- Assist medical providers in delivering high-quality, patient-centered care
- Manage patient flow, including triage, follow-up care, and care coordination
- Coordinate referrals to specialists and community-based services
- Provide health education to patients and caregivers
- Collect and process laboratory specimens (e.g., blood, urine, sputum)
- Support infection control practices, safety standards, and appropriate use of supplies
- Assist with front office functions as needed (scheduling, patient discharge, etc.)
- Float between Community Health Net locations as required.
Qualifications
- Active Pennsylvania Licensed Practical Nurse (LPN) license
- Current CPR certification
- High school diploma or GED
Required Skills & Competencies
- Strong clinical assessment and organizational skills
- Excellent verbal and written communication abilities
- Knowledge of medical terminology and basic insurance types
- Proficiency with computers and electronic health record (EHR) systems
- Ability to work collaboratively in a fast-paced healthcare environment
