Licensed Practical Nurse (Lpn)

Report To: Director of Nursing

Position Summary: The Licensed Practical Nurse (LPN) is responsible for providing nursing 

care to the patients under the direction of a physician or healthcare provider.

Duties and Responsibilities: 

• Collection of selected data about the health status of a patient.
• Administer medications, vaccinations, and treatments as prescribed by provider
• Monitor and record patient vital signs and symptoms, communicating any significant 
• changes to the healthcare team.
• Assist with diagnostic tests and follow up appointments, ensuring patients receive 
• appropriate care.
• Maintain accurate and up-to-date medical records, ensuring confidentiality and 
• compliance with regulatory standards.
• Provide care for each patient in a safe, comfortable, and private environment.
• Provide preventive care information/education that correlates with patient age, sex, and 
• health issues.
• Maintain current knowledge of HEDIS/UDS Measures and review with every patient.
• Assist in wound care, dressing changes, and other routine medical procedures.
• Ensure the cleanliness and organization of patient rooms, equipment, and supplies.
• Adhere to infection control protocols and contribute to maintaining a safe and 
• clean/sterile environment to prevent the spread of diseases.
• Participate in ongoing professional development and training opportunities to enhance 
• nursing skills and company development.
• Provide emotional support and education to patients and their families regarding 
• healthcare procedures and self-care techniques.
• Collaborate with healthcare teams and other healthcare professionals to develop and 
• implement individualized care plans.
• Effective, accurate patient documentation in real time.
• Assist with other duties/activities as assigned.
• Responds to emergency situations
• All other Duties as Assigned

• Ability to communicate effectively in person, by phone and in writing.

• appearance.

• physicians, management and peers.

Requirements: 

• Current State of South Carolina LPN Licensure.
• American Heart CPR certification 
• Experience in a clinical or healthcare setting.
• Strong organization skills and multitasking abilities.
• Proficiency in using electronic medical records (EMR) systems.
• Ability to work in a fast-paced environment, with focus on providing exceptional patient 
• care.

• Physical Requirements:
• Able to perform job duties in person and over the telephone. 
• Must be able to communicate clearly to patients in person and over the Telephone. 
• Requires occasional lifting, upwards of 15lbs, including assisting patients when required.

• Requires frequent bending, reaching, repetitive hand movements, standing, walking, 
• squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly 
• throughout a regular work shift.
