Responsibilities
Provide direct patient care and administer medications per protocols.
Conduct patient assessments and document progress in electronic health records.
Monitor vital signs and respond to changes in patient condition.
Identify and report safety risks and quality-of-care concerns.
Adhere to credentialing, infection control, and regulatory standards.
Coordinate with physicians and interdisciplinary team to plan care.
Maintain accurate charting to ensure compliance and traceability.
Meet productivity targets and contribute to pipeline-based planning.
Participate in ongoing professional development and apply learned skills to practice.
Support patient and family education to promote self-management.
On-site shift coverage with a focus on efficiency and measurable outcomes.
Experience/Qualifications
Licensed Practical Nurse (LPN) licensure in applicable state.
Administer medications per protocols and document administration accurately.
Perform direct patient assessments and document in electronic health records.
Monitor vital signs and respond to changes in patient condition promptly.
Ensure infection control practices and regulatory standards compliance.
Coordinate care with physicians and interdisciplinary team.
Maintain accurate charting for traceability and audit readiness.

Location: Rutland, VermontEmployment Type: Full-Time (36-40 hours/week, day/night shifts available)Pay: $28–$34/hour + shift differentials, sign-on bonus

Key Responsibilities:
Administer medications, IVs, and treatments under RN supervision.
Perform patient assessments, wound care, and monitor vital signs.
Develop and implement basic care plans while educating patients/families.
Collaborate with physicians, RNs, and aides in med-surg, rehab, or long-term care units.
Ensure compliance with state regulations, documentation, and safety protocols.
Requirements:
Active Vermont LPN license (or compact state equivalent).
BLS/CPR certification; IV certification preferred.
Excellent clinical judgment, communication, and teamwork skills.
Ability to lift 50 lbs and work flexible shifts.
