Responsibilities
Provide licensed practical nursing care to patients in on-site settings.
Assess patient needs, monitor vitals, and document changes promptly.
Administer prescribed medications and manage routine treatments per protocols.
Adhere to safety, infection control, and regulatory standards at all times.
Collaborate with healthcare team to develop and adjust care plans.
Maintain accurate and compliant patient records in electronic systems.
Support performance-driven environment by meeting care delivery targets.
Educate patients and families on care plans and post-care instructions.
Provide on-site nursing support with strong accountability for personal outcomes.
Demonstrate eagerness to learn and pursue professional development opportunities.
Contribute to pipeline and territory planning for staffing and workflow.
Experience/Qualifications
Licensed Practical Nurse (LPN) license active in applicable state.
Provide direct on-site nursing care and timely vitals assessment.
Administer medications per protocol and document administration.
Adhere to safety, infection control, and regulatory standards.
Accurate, compliant documentation in electronic health records (EHR).
Collaborate with healthcare team to develop and adjust care plans.
Meet care delivery targets and performance metrics.
Demonstrate eagerness to learn and pursue professional development.
Educate patients/families on care plans and post-care instructions.
Strong accountability for personal outcomes and on-site responsibilities.
