Join our Walk-in Medication for Addiction Treatment (WinMAT) clinics, where we are breaking down barriers to care for individuals with substance use disorders through a low-threshold, team-based model that puts patients first.In this role, you will work at the intersection of nursing excellence and meaningful community impact - delivering protocol-driven clinical care, supporting evidence-based medication treatment (MAT), and serving as a trusted partner on an interdisciplinary care team. What You BringRequired Qualifications

Active, unencumbered Licensed Practical Nurse (LPN) license in the State of Ohio.CPR certification required within 60 days of hire.Nonviolent crisis intervention training required within 60 days of hire.

Preferred Qualifications

Experience working with individuals with substance use disorders.Familiarity with Epic or OCHIN electronic health record systems.Knowledge of harm reduction principles, medication-assisted treatment (MAT), and low-threshold care models.

What You Will Do

Clinical Care & Patient EngagementConduct nursing visits in collaboration with WinMAT providers, RNs, and interdisciplinary care team members, supporting shared visits across the evaluation and management of medication side effects, adherence concerns, and dose stabilization or adjustment.Collect and document objective and subjective clinical data including vital signs, substance use patterns, withdrawal symptoms, psychiatric symptoms, medication adherence and side effects, and safety concerns.Provide individualized patient education on medication for addiction treatment (MAT), treatment plans and expectations, and harm reduction strategies including overdose prevention.Reinforce treatment plans and support adherence using practical, patient-centered strategies - addressing barriers, using behavioral cues, and providing ongoing encouragement.

Clinical Procedures

Perform venipuncture, intramuscular (IM) injections (e.g., IM naltrexone), and subcutaneous injections (e.g., long-acting buprenorphine) within scope of practice and under provider direction.Collect urine and oral toxicology specimens.Administer medications per provider order and document all interventions accurately in the medical record (Epic/OCHIN).Distribute safer use materials, naloxone, and testing strips as indicated.

Care Coordination & Communication

Coordinate with pharmacies regarding medication refills, prior authorizations, and patient assistance programs.Monitor Epic/OCHIN In Basket and message pools to support timely communication across the care team.Conduct protocol-driven phone screenings, gather clinical information, and escalate concerns outside established guidelines to the RN or provider.Collaborate with internal teams, external partners, and patient support systems to facilitate access to care and continuity of treatment.Other ResponsibilitiesPerform other related duties as assigned.
