Licensed Practical Nurse Behavioral Health Visits

Hartford, New Britain & Bloomfield areas

Full Time with lots of benefits- 36 hours- 4 - 9 hours per week, every other weekend.

As a Licensed Practical Nurse Behavioral Health you'll contribute to our success in the following ways:

  • Follow each patient's individualized care plan and physician orders.
  • Monitor patient progress and report changes or safety concerns to the care team.
  • Help patients and caregivers learn self-care techniques to support their health.
  • Work with the RN Case Manager to update care plans as patient needs change.
  • Administer and document medications according to agency guidelines.
  • Communicate with physicians, patients, families, and the care team to ensure coordinated care.
  • Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
  • Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

  • Current State License unrestricted as a Licensed Vocational / Practical Nurse
  • Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
  • 1 year of experience in a clinical care setting
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
  • Experience in a hospice or home health environment is preferred
