Registry personnel must bring a state or federal issued photo ID to the facility and wear a photo badge at all times
Agency staff must have completed the original or updated COVID-19 vaccine series, or present a documented religious and/or medical exemption from their employer, to be considered for any assignment
Additional vaccine doses must be obtained where required by law
Every weekend commitment required
All requests for time off (RTO) must be approved before contract is finalised
Overtime is not available for this assignment
Candidate must have own reliable transportation - public transportation is limited and unreliable in this area

Requirements

Must-Have

Active Vermont LPN/LVN licence - current and in good standing
CPR/BLS certification - current
Minimum 1 year of LPN experience in a Skilled Nursing / Long-Term Care setting
Reliable personal transportation - public transportation in this area is limited and unreliable
