Licensed Social Worker, Lmsw Or Lcsw - Bilingual Spanish/English

First Choice Health Centers is seeking an experienced Bilingual (English/Spanish)Behavioral Health Clinician (Social Worker) to perform initial clinical assessments for individuals seeking behavioral health services. This person will provide clinical social work services, including individual, group, and family therapy. Performs case management services. Provides extensive documentation and care coordination. Participates in the Center's quality assurance, performance improvement and risk management activities as required by Center policies and procedures or as assigned by Center leadership. This position is located at 92 CT Blvd, East Hartford, CT with opportunity for hybrid work.

• Primarily a Monday through Friday schedule working 8:00 am to 4:30 pm. There is one late night until 6 pm with an offset of leaving on Friday at 3 pm.
• The budgeted pay range for this position is $62,000 - $80,000. Actual pay will be determined based on several factors. These may include education, work experience, and in some instances certifications and licensure. We strive for market alignment and internal equity with our colleague's pay.

Minimum Knowledge, Skills & Abilities Required:

• Master's Degree from an accredited school of Social Work
• Licensed to practice Social Work in the state of Connecticut

Experience and Training:

• Minimum of one (1) year of experience in behavioral health, mental health, or social services preferred.
• Experience conducting clinical assessments and working with diverse populations.
• Experience in outpatient behavioral health, community mental health, or integrated care settings.
• Experience with crisis intervention and risk assessment.
• Bilingual Spanish/English preferred
• Familiarity with electronic health records and managed care requirements.

Standard Job Duties:

• Compile and complete assessment information in documenting comprehensive and relevant case formulations, and treatment planning
• Conduct ongoing risk assessments (i.e., SI, HI)
• Develop treatment plans that include observable and measurable goals and modify as necessary
• Integrate clients' needs, strengths, and barriers to treatment
• Provides individual therapy services to patients.
• Provides group therapy services to patients
• Provides family therapy services to patients
• Perform de-escalation techniques while maintaining a safe and therapeutic environment
• Provides extensive documentation of the case, including (based on program needs) Social History, progress notes and Treatment via electronic health record
