Photo ID and facility badge required at all times

Key Responsibilities

Administer medications and treatments per physician orders and facility policy
Perform wound care, catheter care, and other skilled nursing procedures
Complete nursing assessments and contribute to care plan updates
Document accurately in the EMR and communicate patient status changes
Supervise CNA/GNA staff as assigned
Support infection control, regulatory compliance, and quality care initiatives

Requirements

Active state LPN/LVN licence (or compact licence eligibility)
CPR/BLS certification current
Minimum 1 year of LPN experience in a LTC/SNF or similar setting
Proficiency with EMR documentation and medication administration
Strong communication skills and ability to work within a multidisciplinary team
