Agency staff must have completed the COVID-19 vaccine series or present a documented religious/medical exemption from their employer

Photo ID and facility badge required at all times

Every other weekend rotation

2-week rotating schedule - confirm full pattern at offer

Key Responsibilities

Provide direct personal care including bathing, grooming, dressing, and mobility assistance
Monitor and report changes in resident condition to nursing staff
Assist with meals, feeding, positioning, and range-of-motion activities
Document care accurately in the EMR per facility policy
Maintain a safe, clean, and dignified environment for all residents
Support infection control protocols and regulatory compliance

Requirements

Active state LNA/CNA certification in good standing (VT or NH)
CPR/BLS certification current
Minimum 6 months of experience in a LTC/SNF or similar care setting preferred
Reliable and punctual with ability to commit to a rotating schedule
Ability to work as part of a nursing team
