Summary: General anesthesia MD

BID DUE DATE: 05/07/2026; 5pm EST

RATES: (strictly follow)

Malpractice Must be included in rates.

Orientation -262.50 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly -525- Per hour, not OT

Holiday -787.50 - Must be 1.5x the hourly rate Per holiday hour, no OT

Nightly Call $-- Per night call to hold the pager, to include 2.5 hours gratis

Holiday Nightly Call - -3150 - "Per night call to hold the pager, to include 2.5 hours gratis

On Call (Daily) -4500- Per 24 hours to include 4 hours gratis"

24 Hour Holiday Call Rate - -- Per 24 hours to include 4 hours gratis"

Call Back $-525 Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.

Holiday Call Back $-787.50 Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.

Average Callback: Weekday: 7.5 hours; Weekend: 5.9 hours

weekdays and weekend coverage needed

Occasional OR coverage; 7a- 3p or 7a- 5p

Travel Preferences:

Preferred Lodging: Preferred hotels

Air Travel: Local, no flights?
