Job Description
LPN / RN - Home Health Care Nurse We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) for a private duty home care case in Mertztown, PA. This position involves providing skilled nursing care and hands-on support for a patient with ongoing respiratory and personal care needs in the home setting.
Responsibilities:
- Monitor oxygen levels and respiratory status
- Administer breathing treatments and prescribed medications
- Provide skilled nursing assessments and ongoing patient monitoring
- Assist with turning and repositioning to maintain comfort and prevent skin breakdown
- Monitor and maintain skin integrity
- Provide incontinence care as needed, including changing briefs and ensuring clean bedding
- Maintain accurate documentation of care provided and patient status
- Communicate changes in condition to the appropriate parties
Requirements:
- Active Pennsylvania LPN or RN license
- Experience with respiratory care and oxygen monitoring preferred
- Comfortable providing total care and hands-on assistance in a home setting
- Reliable, compassionate, and attentive to patient needs
- Current CPR certification preferred
Schedule:
- Friday through Sunday
- 3:00 PM 11:00 PM
