We are currently seeking a compassionate and skilled LPN (Licensed Practical Nurse) to join our home health team.

Responsibilities
- Provide direct patient care under the supervision of a registered nurse.
- Administer medications and treatments as prescribed by physicians.
- Monitor and record patients' vital signs and health status.
- Assist with daily living activities, including bathing, dressing, and grooming.
- Educate patients and their families about health management and home care.
- Maintain accurate and timely patient records and documentation.
- Communicate effectively with healthcare team members to ensure optimal patient care.
Requirements
- Current LPN licensure in the state of practice.
- Valid CPR and BLS certification.
- Previous experience in home health care or a similar setting is preferred.
- Strong assessment and critical thinking skills.
- Ability to work independently and manage time effectively.
- Excellent communication and interpersonal skills.
- A genuine compassion for patients and a commitment to quality care.
