Job Description

Job Summary:

The LPN provides direct patient care and assists with nursing care plans under supervision. Responsibilities include medication administration, nursing procedures, patient support, and record-keeping. The LPN strong clinical skills and a commitment to high-quality care can make a difference in our senior patient lives every day.

Essential Duties and Responsibilities

  • Supports and upholds the mission and values of Covenant Health Ministry.
  • Uses adaptive equipment as indicated for personal hygiene, mealtime, mobility.
  • Assists with personal hygiene tasks such as bathing, dressing, oral care, hair care.
  • Provides support with ambulation, mobility, and positioning, ensuring proper transfers to and from bed, chair, and toilet using correct body mechanics.
  • Assists during mealtimes by preparing the patient, serving meals and snacks and offering fresh water.
  • Assists with toileting, which may involve accompanying to the bathroom, using a commode, or providing incontinent care.
  • Conducts patient observations and gathers data to aid the Professional Partner in the assessment process.
  • Executes specified patient procedures, tests, and treatments within the scope of practice and according to established policies and procedures.
  • Promptly reports patient outcomes to the Professional Partner.
  • Maintains accurate and precise documentation in the patient record from admission through discharge, including care provided, education delivered, observations made, and patient responses.
  • Follows appropriate communication channels, providing concise, relevant information to fellow healthcare team members and reporting changes in patient status promptly.
  • Administers medications and IV therapy in accordance with organizational protocols.
  • Monitors blood transfusions and promptly communicates any issues to the Professional Partner.
  • Actively participates in the established plan of care and provides transport as needed.
  • Ensures a clean and safe environment, adhering to infection control principles, responding promptly to call lights, and changing linens as required.
  • Assists patients with the care of personal belongings and educates patients and families on diagnosis-specific home care plans and self-care expectations.
  • Assesses the level of patient/family comprehension of teaching and adjusts methods to maximize understanding.
  • Reviews discharge instructions with patients and families upon development by the Professional Partner.
  • Engages in stretch breaks and utilizes appropriate safety equipment for transferring and repositioning elders/patients, including wearing and utilizing a gait belt.
  • Attends mandatory Safety and Prevention educational sessions and promotes safe practices.
  • Demonstrates understanding of organizational goals and articulates the importance of safety and prevention.
  • Maintains required licenses, certifications, and competencies, and completes annual compliance courses on time.
  • Supporting student learning is a shared responsibility. Every team member is expected to contribute to creating a welcoming environment and to actively assist students in their educational experience, regardless of their specific role.
  • All other duties as assigned.

Job Requirements

Job Knowledge and Skills

  • Customer service, interpersonal, and organizational skills.
  • Knowledge of Geriatric care desired.
  • Must be able to fluently speak, read, write, and comprehend the English language.
  • Follow verbal and written instructions.
  • Flexibility required to provide coverage 24 hours/day, 7 days per week. May be asked to float to other units.
  • Computer skills &
  • Mathematical skills.
  • Medical Terminology required.
  • Ability to multi-task and make decisions under pressure.
  • Work alone and as part of a team.

Education and Experience

  • 0-1 year of experience working with Geriatrics patients desired.
  • IV Certification preferred.
  • Valid State of Maine Licensed Practical Nurse.
  • Active BCLS

    Comp Range:

    $28.88 - $39.77
