Responsibilities
Provide patient care under supervision, documenting observations and outcomes accurately.
Administer medications and treatments following protocols and safety guidelines.
Maintain accurate and timely patient records for compliance and auditing.
Identify and escalate clinical concerns promptly to supervising clinician.
Ensure compliance with healthcare regulations, privacy laws, and facility policies.
Assist with patient assessments and vital signs to support care planning.
Collaborate with a multidisciplinary team to achieve patient care goals.
Participate in ongoing professional development and skill enhancement.
Demonstrate reliability in a results-driven environment with transparent performance metrics.
Support goal-oriented targets through efficient workflow and time management.
Engage in quality improvement initiatives to optimize patient care processes.
Provide compassionate patient and family education on treatments and care plans.
Experience/Qualifications
Active LPN license in the applicable state or region.
Prior patient care experience in acute or ambulatory settings.
Ability to document observations and outcomes accurately and promptly.
Medication administration competency following safety protocols.
Strong understanding of privacy laws and healthcare compliance standards.
Proven ability to work under supervision within a multidisciplinary team.
Demonstrated reliability in a metrics-driven, target-focused environment.
Experience with patient assessment, vital signs, and care planning.
Commitment to ongoing professional development and skill enhancement.
Excellent communication skills for patient and family education.
