Responsibilities
Provide patient care under supervision, ensuring safety and comfort.
Monitor vital signs and document changes in patient condition promptly.
Support care plans by administering prescribed treatments and medications.
Adhere to regulatory and safety standards; ensure infection control compliance.
Maintain accurate, timely documentation in patient records and logs.
Collaborate with healthcare team to identify escalating patient needs.
Demonstrate accountability for personal performance within a team-driven environment.
Participate in performance metrics and quality improvement activities.
Educate patients and families on care procedures and post-discharge steps.
Utilize care pipelines and territory insights to anticipate patient needs.
Support the development of care plans aligned with growth and merit-based advancement.
Experience/Qualifications
Licensed Practical Nurse (LPN) with active state license.
Adhere to infection control standards and regulatory requirements.
Provide patient care under supervision with emphasis on safety and comfort.
