Responsibilities

  • Provide licensed practical nursing care to patients following care plans.

  • Assess patient needs and document observations accurately for care teams.

  • Administer medications and monitor for adverse reactions per protocols.

  • Adhere to regulatory standards and company policies at all times.

  • Coordinate with multi-disciplinary teams to ensure timely patient progress.

  • Maintain accurate, up-to-date patient records and eligibility for services.

  • Meet productivity targets and contribute to pipeline-based performance metrics.

  • Support quality improvement initiatives to increase patient outcomes.

  • Educate patients and families on care plans and post-discharge steps.

  • Maintain a high-visibility work ethic by documenting daily activities and outcomes.

  • Assist with onboarding and mentoring of new staff to foster growth.

Experience/Qualifications

  • Valid LPN license in applicable state.

  • Experience with medication administration and adverse reaction monitoring.

  • Ability to accurately document observations in electronic health records.

  • Current CPR/ACLS certification preferred or required.

  • Familiarity with care plans and regulatory standards (e.g., HIPAA, OSHA).

  • Strong collaboration with multidisciplinary teams to coordinate care.

  • Commitment to quality improvement initiatives and patient outcomes.

  • Demonstrated productivity and contribution to pipeline-based targets.

  • Excellent patient and family education skills for discharge planning.

  • Willingness to mentor and onboard new staff to support growth.

  • On-site availability for contract role with high visibility and daily activity documentation.
