Responsibilities
Provide direct patient care and support under the supervision of registered nurses.
Engage in continuous learning and professional development opportunities.
Administer medications and monitor patient responses to treatments.
Adhere to safety and infection control protocols at all times.
Utilize performance metrics to track personal and team success.
Assist in developing and implementing patient care plans.
Maintain accurate patient records and documentation.
Monitor vital signs and report any changes to the nursing staff.
Communicate effectively with patients and their families regarding care.
Participate in team meetings to discuss patient progress and care strategies.
Assist with daily living activities to enhance patient comfort.
Experience/Qualifications
Current LPN license in the state of practice.
Strong commitment to continuous learning and professional development.
Experience in direct patient care, preferably in a clinical setting.
Excellent communication skills with patients and families.
Willingness to engage in performance metrics and accountability.
Ability to administer medications and monitor patient responses accurately.
Familiarity with safety and infection control protocols.
Ability to work collaboratively in a team environment.
Proficient in maintaining accurate patient documentation.
Experience in developing patient care plans is a plus.
