Job Title: LPN Medical Records NurseMarshfield Care Center – Marshfield, MO
Position Summary:The LPN Medical Records Nurse is responsible for maintaining accurate, complete, and compliant clinical records for all residents in accordance with federal, state, and facility regulations. This role ensures medical records are properly organized, updated, and readily accessible while supporting the clinical team in documentation standards and audits.
Key Responsibilities:
- Maintain and organize resident medical records (electronic and/or paper)
- Ensure all documentation is accurate, complete, and compliant with regulations
- Perform routine chart audits and follow up on missing or incomplete documentation
- Manage admissions, discharges, and record retention processes
- Assist with MDS and care plan documentation as needed
- Prepare records for surveys, audits, and inspections
- Collaborate with nursing staff to ensure timely and accurate charting
- Maintain confidentiality in accordance with HIPAA guidelines
Qualifications:
- Active Missouri Licensed Practical Nurse (LPN) license
- Previous experience in long-term care preferred
- Knowledge of medical records management and healthcare regulations
- Strong attention to detail and organizational skills
- Experience with electronic medical records (EMR) systems preferred
- Ability to work independently and as part of a team
