Overview
Job Description
- Manage medication ordering, delivery, storage, and administration per program guidelines.
- Educate individuals and staff on health concerns; provide staff training as needed.
- Assess and respond to new or ongoing health issues.
- Complete required documentation for RHD and regulatory bodies.
- Conduct screenings for depression, trauma, substance use, and criminal risk using validated tools.
- Provide recovery-oriented, evidence-based treatment and collaborative documentation.
- Act as liaison for medical care and community supports.
- Coordinate care with prescribers and other providers; administer IM injections per physician orders.
- Monitor vital signs and medication side effects; report findings to the team.
- Ensure consumers have primary care providers, annual physicals, dental exams, and prescription schedules.
- Participate in on-call rotation for 24-hour emergency and crisis intervention.
- Develop recovery plans and recommend health objectives.
- Deliver culturally competent, individualized services.
- Perform other duties as assigned by the Team Leader/Program Director.
- Licensed Practical Nurse (LPN) license.
- Current license to practice in the State of Delaware.
- Valid driver’s license.
- Ability to lift up to 50 pounds occasionally (medium physical demand).
- Comfortable working in varied conditions (e.g., exposure to heat, cold, dust, fumes, confined spaces).
- Physical abilities include standing, sitting, walking, climbing, reaching, kneeling, and repetitive motion.
- Strong communication and documentation skills.
