What You’ll Do • Provide skilled, one-on-one nursing care in the home • Administer and manage medications • Monitor and document patient conditions • Perform treatments as ordered by the physician • Educate and support family members • Collaborate closely with the Phoenix clinical team

What You Need• Active IL RN or LPN license (or Compact license)• Strong assessment and communication skills• Compassionate, dependable, and self-motivated• Valid driver’s license and reliable transportation
