Job Title: LTSS Service Care Manager- J01031
Location: Remote and visiting members at their homes in Delaware counties
- 3 needed in New Castle County
- 2 needed in Kent County
- 1 needed in Sussex County
Shift:
- Monday-Friday 8am- 5pm
- No overtime expectations
Duration:
- 6 months, with possible extension or conversion
Description:
- Assist in developing, assessing, and coordinating holistic care management activities
- Enable quality, cost-effective healthcare outcomes
- Develop or assist with personalized service care plans for long-term care members
- Educate members and their families/caregivers on available services and benefits
Candidate Requirements:
- Education/Certification: Required: Bachelor's degree and 2 - 4 years of related experience
- Licensure: Required: Delaware/Compact RN License, Driver's license
- Must have a valid Driver's license and home workspace
- Nice to have: Case Management Experience, Service Care Manager
- Disqualifiers: Must be located within Delaware
Performance Indicators:
- 4-5 assessments per week
- For redeterminations: 4-5 daily
