Job Description
Location: DelawareWork Mode: Hybrid (mixture of care visits and remote work)Shift: 8am – 5pm EST Mon – Fri, No OT expectationsDuration: 6 months, potential to extend or convert
Responsibilities:
- Complete LTSS level of care redetermination
- Conduct a mixture of care visits and support other LTSS case managers
- Support the case management team due to a high increase in membership
Requirements:
- Bachelor's degree
- Minimum 2 years of related experience
- Delaware/Compact RN License
- Driver’s license
- Home workspace
- Experience in Level of Care Redetermination, Transition of Care, and Case Management
- Computer literacy
- Experience evaluating the level of functioning of older adults/disabled individuals
- Experience with medication reconciliation
Preferred Skills:
- Service Care Manager experience
