Summary: Work Mode: Remote and Field Visits are required throughout New Castle, mileage will be reimbursed by the company. Duration: 6 months, possible extension or conversion. Schedule is planned from Monday-Friday 8am – 5pm | No OT expectations Responsibilities: Complete level of care redetermination. Conduct a mixture of in-home care visits. Support other case managers. Assist the case management team due to high increase in membership. Requirements: Bachelor's degree and 2 – 4 years of related experience. Delaware/Compact RN License. Driver’s license. Must have a home workspace. Experience with Level of Care Redetermination, Transition of Care, and Case Management. Computer literate. Experience in evaluating older adults/disabled level of functioning. Experience with medication reconciliation. Disqualifiers: Location should be within Delaware. Performance Indicators: 4-5 assessments per week for redeterminations. 4-5 daily assessments. Preferred Skills: Service Care Manager experience. AXEL01
