Job Title: LTSS Service Care Manager - J01031
Location: Remote with visits to Delaware counties
- 3 needed in New Castle County
- 2 needed in Kent County
- 1 needed in Sussex County
Pay: Not specified
Shift:
- Monday-Friday, 8am 5pm
- No overtime expectations
Duration:
- 6 months, with possible extension or conversion
Description:
- Assist in developing, assessing, and coordinating holistic care management activities
- Enable quality, cost-effective healthcare outcomes
- Develop or assist with developing personalized service care plans for long-term care members
- Educate members and their families/caregivers on services and benefits available

- Education/Certification:
- Required: Bachelor's degree and 2 4 years of related experience
- Preferred: Not specified
- Licensure:
- Required: Delaware/Compact RN License, Driver s license

- Must haves: Valid Driver s license, Home workspace
- Nice to haves: Case Management Experience, Service Care Manager experience
- Disqualifiers: Location should be within Delaware
- Performance indicators: 4-5 assessments per week, for redeterminations 4-5 daily
