Job Description

Location: New Castle CountyWork Mode: Mixture of in-home care visits and remoteDuration: 6 months contractSHIFT: 8am – 5pm EST Mon – Fri, NO OT

Responsibilities:

  • Complete LTSS level of care redeterminations.
  • Conduct a mixture of care visits and support other LTSS case managers.
  • Assist the case management team due to high increase in membership.
  • Perform transition of care and case management duties.
  • Visit member homes as required.

Requirements:

  • Bachelor's degree and minimum 2 years of related experience.
  • Delaware/Compact RN License.
  • Driver’s license.
  • Home workspace.
  • Experience in Level of Care Redetermination, Transition of Care, and Case Management.

Preferred Skills:

  • Service Care Manager experience.
  • Computer literate.
  • Experience evaluating older adults/disabled level of functioning.
  • Experience with medication reconciliation.
