Job Description

Summary

Under the general supervision of the Practice Manager and with direction from Pulmonary Physicians, the Lung Nodule Nurse Navigator serves as a clinical resource and care coordinator for patients identified with pulmonary nodules. The navigator facilitates timely evaluation, diagnosis, treatment, and follow-up while providing education and support to patients and families. This role works closely with pulmonologists, thoracic surgeons, radiology, oncology, and primary care providers to ensure adherence to evidence-based guidelines and streamline care across the system.

The Lung Nodule Nurse Navigator is responsible for tracking patients with pulmonary nodules, assisting in coordinating diagnostic testing and procedures, and reducing delays in care.

Education

Graduate from an accredited school of Nursing, BSN required.

Certification, Registration & Licensure

Licensure required: Licensed as a Registered Nurse in the State of New Hampshire.

Experience

Minimum 3 years of nursing experience. At least one year of experience in pulmonary, oncology, thoracic surgery, or intensive care is strongly preferred. Experience with lung cancer screening or pulmonary nodule programs and familiarity with guidelines for pulmonary nodule management (e.g., Fleischner Society) preferred.

Responsibilities

  • Identify screening detected lung nodules and perform clinical intake and chart reviews.

  • Facilitate timely diagnostic workups, including imaging, biopsies, and specialty consultations.

  • Serve as the primary, consistent point of contact for patients and families from initial diagnosis through treatment and follow up care.

  • Provide specialized education on pulmonary nodules and procedures while connecting patients with necessary resources and support services.

  • Monitor patients for appropriate surveillance intervals according to evidence-based guidelines.

  • Lead communication and participation in multidisciplinary case conferences.

  • Collaborate with oncology and radiology to maintain a comprehensive lung nodule tracking database or registry.

  • Support accreditation requirements and assist in the development of clinic workflows, protocols, and lung cancer screening initiatives.
