Job Description
Job Summary:
- Experience Level: Mid-Senior (5+ to 7 years total; 6 years Case Management; 2 years supervisory/management).
- Education: Master's degree in business, social work, nursing, or healthcare-related field (minimum Bachelor's noted).
Key Responsibilities:
- Manage daily operations and staff in Case Management Department including care coordination, discharge planning, utilization management, social work, and pre-admission services.
- Act as liaison between patients, families, staff, physicians, healthcare team, community agencies, managed care, and skilled nursing facilities.
- Establish, implement, and monitor best practices and clinical standards for quality patient care and organizational financial integrity.
Required Qualifications:
- Master's degree from accredited school in business, social work, nursing, or healthcare-related field.
- Washington Registered Nurse License upon hire.
- 6 years experience in Case Management.
- 2 years supervisory/management experience, preferably in Case Management.
- Previous nursing experience in complex client care areas.
- Experience in problem-solving, conflict resolution, and adult education for multi-disciplinary staff.
- Preferred Qualifications:
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) upon hire.
- Case Management and supervisory experience in acute care hospital.
