Community Care Network is looking for a Manager - Shared Living to join our team!
Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through a responsive, innovative and collaborative human service
About the Role:
The Manager of Shared Living is responsible for overseeing the day-to-day operations, compliance, and quality of the Shared Living program while serving as the primary point of contact for Shared Living Providers. This role ensures providers are supported with clear expectations, training, and ongoing guidance, while also maintaining adherence to regulatory, documentation, and program requirements. It involves close coordination with clinical, case management, and operational teams to support placements, manage service changes, address concerns, and ensure consistent communication across all stakeholders.
In addition, the Manager leads provider recruitment, onboarding, and retention efforts, monitors program quality through audits and incident review, and supports financial and operational processes such as provider payments, authorizations, and capacity planning. The role is focused on maintaining strong provider relationships, ensuring compliance and quality standards, and continuously improving outcomes for individuals served within the Shared Living program.
Responsibilities:
Shared Living Provider Oversight
- Serve as the primary contact for Shared Living Providers
- Ensure Residential Inspections are completed
- Monitor provider performance and satisfaction
- Support SLP's with problem-solving and crisis coordination
- Ensure providers understand expectations, policies, and compliance requirements
Compliance and Documentation
Monitor compliance with:
DDS Medicaid Manual requirements
Service documentation expectations
EVV/timekeeping requirements (if applicable)
DOL and wage/hour concerns
Review monthly summaries and provider documentation for quality and completeness
Coordinate corrective action plans when needed
Coordination with Clinical & Case Management Teams
Work closely with care managers and program staff regarding:
Variance requests
Behavioral or medical concerns
Service changes
Transitions and placements
Participate in team meetings and ISP-related planning as needed
Help ensure consistency in communication across teams
Recruitment and Retention
- Recruit new Shared Living Providers
- Conduct interviews, orientation, and onboarding
- Develop retention strategies and provider supports
- Coordinate respite coverage planning
Quality Assurance
- Monitor incidents and trends within Shared Living
- Conduct audits of files and documentation
- Track outcomes and satisfaction
- Identify program improvement opportunities
Financial & Operational Coordination
Coordinate with payroll/billing regarding:
Provider stipends/payments
Contracted supports concerns
Authorization changes
Monitor vacancies and placement capacity
Assist leadership with forecasting provider and staffing needs
Other duties as assigned.
Qualifications:
- Bachelor's degree in human services, social work, psychology, healthcare administration, or related field preferred; equivalent combination of education and experience considered
- 3-5 years of experience in intellectual/developmental disabilities (I/DD), residential services, or community-based programs
- Experience in shared living, case management, quality assurance, or compliance strongly preferred
- Knowledge of Medicaid-funded services and applicable state/federal regulations
- Strong understanding of residential service delivery and shared living models
- Knowledge of compliance, documentation standards, and quality assurance practices
- Ability to build and maintain effective provider and stakeholder relationships
- Strong communication, coaching, and conflict resolution skills
- Demonstrated ability to manage multiple priorities and maintain attention to detail
- Analytical skills with the ability to identify trends, risks, and improvement opportunities
- Collaborative approach with cross-functional teams including clinical, billing, and operations
- Ability to work independently while exercising sound judgment and decision-making
- Must have a valid driver's license, and reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.

Competitive Salary $63,000.00-$70,500.00 (compensation is typically dependent upon experience)
