Job Summary

Responsible for the execution of the clinical aspects of the Case Management process. Provides outreach and enrollment services to meet the eligibility requirements of the program and the surrounding counties. Ensures coordination and planning with the help of community and family support. Focuses on the development and coordination of community service plans. Partners with service providers, families, and patients to create a plan of healthcare and identify additional service providers. Responsible for overseeing and supervising staff, and serving as a role model for staff. Fully responsible for individual and team assignment of patients in a timely manner. Participates with leadership in the development of the case management standards, evaluation of outcomes, and development of improvement initiatives.

Primary Responsibilities:

  • Provide day-to-day supervision to Case Managers and interns/students as appropriate.
  • Ensure staff complete psychosocial assessments covering psychological, social and environmental concerns, including evaluation of mental status.
  • Review daily referrals and screenings of patients
  • Audit case file on a regular basis for completeness and accuracy.
  • Assist in discharge planning as required.
  • Serves and protects the care management program by adhering to professional standards, care management policies and procedures, federal, state, and local requirements, and NCQA compliance.

Qualifications

Qualifications:

  • Bachelor's Degree Nursing required or Master's Degree Social Work required

  • Licensed Independent Clinical Social Worker (LICSW) State License in MA and NH required or Registered Nurse (RN) State License in MA and NH required

  • 3+ years supervisory or management experience with professional staff in a healthcare setting required

  • 3+ years of experience in case management, utilization review, or discharge planning required

  • Ability to deal with rapid changing health care environment and Requires excellent interdisciplinary team collaboration skills.

Additional Job Details (if applicable)

Schedule and Work Model:

  • Full-time, Monday through Friday, standard business hours.

  • Hybrid work Model: Approximately 2-3 days per week on-site at Wentworth Health Partners practice sites in New Hampshire and southern Maine and approximately 2-3 days remote from home.

  • On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams Video is required and must be accessed using MGB-provided equipment.
