Job Description

START Clinical Coordinator
Summary:START (Systemic, Therapeutic, Assessment, Resources and Treatment) works collaboratively with the system of support to improve the lives of individuals with intellectual/developmental disabilities and co-existing behavioral health needs/mental health concerns. The Clinical Coordinator is responsible for providing supportive services to people receiving assistance through the START Program.Essential Duties and Responsibilities:
  • Maintain an active caseload of people referred for START services (variability occurs depending on the activity level of people supported and the project scope).  
  • Demonstrate competencies in all areas associated with START coordination.
  • Understand how to use Ecomaps as part of systemic analysis and provide consultation to systems/teams as needed using best practices. 
  • Assist with referrals for START services as needed, including screening for enrollment in START.
  • Ensure the coordination of support meetings and crisis plan development for people enrolled in START.
  • Ensure all systems receive training on all START support plans on a regular basis (updates, new systems partners, etc.).
  • Provide outreach visits to the person’s home, day and vocational settings regularly.
  • Use the START Action Plan for planning and strategizing.
  • Develop Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, admission/discharge summaries and any other applicable documentation of services provided.
  • Develop materials and present at Clinical Education Team Meetings as assigned by the director or designee.
  • Complete all required data entry expectations within outlined timeframe.
  • Provide rotating on-call support as part of the START team.
  • Knowledge of PERMA+, strength-spotting and evidence-based assessment provided by the START Team.
  • Participation in daily Triage calls with START team and external partners.
  • Knowledge of application regulation requirements for the systems involved with enrollees.
  • Participating in the Center for START Services National Network, which brings together projects from across North America through an online resource center, conference calls, select study groups and a meeting at an annual conference is recommended.
Supervisory Responsibilities:This job has no supervisory responsibilities. Additional Responsibilities:From time to time the employee will be required to perform additional tasks and duties as required by the employer.        Knowledge, Skills and Abilities:
  • Ability to use various computer programs (i.e. MS Word, Outlook, Excel, etc.).
  • Demonstrate creative problem-solving skills that promote maximum effectiveness of both natural and community resources.
  • Demonstrated excellence in customer service by resolving issues promptly and maintaining high levels of satisfaction.
  • Demonstrate an ability to facilitate, plan, and organize muti-system activities and handle multiple priorities simultaneously.
  • Excellent written and verbal communication skills when presenting to senior management, stakeholders and system partners.
  • The ability to work with complex systems and an interest in the population is essential.
  • Ability to work independently and as part of a cohesive treatment team.
  • Organization and communication skills are essential.
  • Experience working with people and their families/systems.
Education/Experience Requirements:
  • A master’s degree in social work, psychology, counseling, or other human services field.
  • At least 2-years’ experience working with people with ID/A and behavioral health needs.
  • Must complete START Coordinator certification within 12 months following date of hire.
for some START programs, a bachelor's degree in social work, psychology, counseling, or other human service field with 5-years’ experience working with people who have ID/A and behavioral health needs is acceptable.  This is an exception to the START program and leadership should work collaboratively with the Center for START services to determine if appropriate or necessary. Certificates, Licenses, Registrations:Act 33 and 34 clearances.-AND-Valid driver’s license and access to a reliable vehicle.This position will be formally employed by Community Family Advocates. In this capacity, the individual will report directly to Allegheny County Department of Human Services and will carry out day-to-day responsibilities under the direction, supervision, and operational guidelines of the Allegheny County Department of Human Services. While Community Family Advocates will serve as the official employer for administrative and human resources purposes, the role will function as an integrated part of Allegheny County’s team.
