  • Coordinate and complete MDS assessments accurately and within required timeframes
  • Manage the RAI/MDS process in accordance with CMS and state regulations
  • Develop and maintain individualized resident care plans in collaboration with the interdisciplinary team
  • Monitor assessment schedules and ensure timely completion of required documentation
  • Review clinical documentation for accuracy, completeness, and appropriate reimbursement
  • Support Medicare and Medicaid reimbursement processes, including PDPM
  • Participate in care plan meetings and communicate with residents, families, and members of the care team
  • Monitor changes in resident condition and ensure assessments and care plans are updated appropriately
  • Maintain compliance with federal, state, and facility standards
  • Assist with audits, surveys, and quality improvement initiatives
  • Serve as a resource to clinical staff regarding MDS documentation and regulatory requirements
Qualifications:
  • Active nursing license or ability to obtain one
  • Previous MDS experience in a skilled nursing or long-term care setting preferred
  • Strong understanding of the RAI process, MDS 3.0, Medicare/Medicaid guidelines, and PDPM
  • Strong organizational skills and attention to detail
  • Ability to manage deadlines and multiple assessments simultaneously
  • Excellent communication and interdisciplinary collaboration skills
  • Knowledge of long-term care regulations and clinical documentation requirements
