What You'll Do
As our MDS Coordinator, you will oversee and coordinate the full resident assessment process in compliance with federal and state regulations. Your work will ensure accurate reimbursement, regulatory compliance, and exceptional resident-centered care.
You will conduct comprehensive MDS assessments, assign ICD-10-CM diagnosis codes, and manage Medicare Part A and insurance case management. You'll collaborate closely with nursing staff, therapists, and interdisciplinary teams to develop and maintain individualized, resident-driven care plans.
In this role, you will:
- Set and monitor assessment schedules to ensure timely and accurate completion
- Transmit assessments to the Department of Health and review outcomes
- Make policy and procedural recommendations to leadership
- Coordinate Medicare and Medicaid schedules and coverage
- Initiate Medicare coverage for qualified residents and issue denial letters when appropriate
- Stay current with regulatory changes affecting Medicare and Medicaid reimbursement
- Ensure documentation accuracy and compliance with MDS 3.0 standards

What We're Looking For
We're seeking a clinically strong and detail-oriented RN who combines analytical skill with empathy and teamwork.
Qualifications:
- Active Registered Nurse (RN) license in good standing
- Associate degree required; Bachelor's degree preferred
- Minimum one year of MDS experience in a skilled nursing or long-term care setting
- Extensive knowledge of MDS 3.0
- Resident Assessment Coordinator (RAC) certification preferred
- Working knowledge of state and federal regulations
- Strong communication, organization, and leadership skills
