About the Position We are seeking a caring and detail-oriented Registered Nurse to join our Adult Foster Care (AFC) program as an Initial Assessment / Admission Nurse. In this role, you will complete in-home evaluations for individuals applying to the AFC program, reviewing their needs and confirming eligibility. This opportunity is ideal for nurses with experience in geriatrics, home health, or community-based care who are committed to supporting individuals and enhancing quality of life.
Key Duties
- Conduct comprehensive in-home assessments for new AFC applicants.
- Evaluate medical, functional, and psychosocial conditions to determine eligibility.
- Partner with caregivers, families, and healthcare professionals to design customized care plans.
- Provide education to participants and families regarding AFC services, expectations, and program policies.
- Ensure compliance with all state regulations and AFC program requirements.
- Maintain accurate, up-to-date documentation of assessments, care plans, and follow-up activities.
