In order to do that, we have designed an innovative health care model for families and individuals living with type 2 diabetes. This model combines best-in-class therapies and technology with day-to-day support, coaching, and analytics to help patients to achieve extraordinary outcomes.

What you’ll do

We’re hiring an experienced Medical Assistant - referred to as a Health Coach at Ryse - to support patients virtually and in-person at our current and future clinics. You’ll build deep relationships with our patients and support them on their journey to better health while working closely with our licensed clinicians. Day-to-day you will:

  • Empower patients to reach their health goals by using motivational interviewing techniques and continuous glucose monitor (CGM) data
  • Collaborate with the Ryse Health team on care planning and support patients in the successful implementation of care plans across the domains of diet, exercise, sleep, stress management, and medication taking
  • Communicate with patients through video calls, in person meetings, telephone and messaging
  • Collect critical patient data including vital signs, A1c, and retina scans
  • Educate patients, including using CGMs and medications and how to treat hypoglycemia
  • Other activities to support patients to better cardiometabolic health in the context of team-based care

About You

  • 1+ years of experience in a clinical setting
  • Motivational interviewing training, phlebotomy or Medical Assistant certification and experience a plus
  • Self-motivated and adaptable to changing processes
  • Ability to balance multiple competing priorities, and keeping track of the little details
  • Passion for the impact of your work on the broader world, and excitement to help reduce the burden of chronic disease
