We are recruiting for a Full-Time Medical Day Care Social Worker to join our skilled team of pediatric professionals in Northfield, NJ. This position is benefit eligible.

POSITION SUMMARY:
The Medical Day Care Social Worker (SW) provides comprehensive psychosocial assessment, care coordination, and family support services to pediatric patients and their families.
The Social Worker is a key member of the Interdisciplinary Team (IDT) and is responsible for:
- Conducting psychosocial assessments
- Developing and contributing to individualized care plans
- Facilitating family involvement in care
- Providing education regarding disability and care needs
- Coordinating community resources
- Providing counseling, home visits, and discharge planning
The SW supports the emotional, social, and financial well-being of children and families while ensuring compliance with:
• Facility policies and procedures • NJ Department of Health Pediatric Medical Day Care standards • Interdisciplinary care planning requirements • Confidentiality and regulatory standards
POSITION QUALIFICATIONS:
- Licensed or certified by the New Jersey Board of Social Work Examiners
- Minimum one (1) year experience in pediatric or family social work
- Strong communication and interdisciplinary collaboration skills
- Computer proficiency (EMR, Microsoft Office, documentation systems)
- Knowledge of community resources and support systems
- CPR Certification (BLS)
