Community Bridges is seeking one (1) part-time Medical Direct Support Professional (DSP) to join our team in Northfield, NH.
Founded in 1982, Community Bridges is a nonprofit organization dedicated to advancing the integration, growth, and interdependence of people with developmental disabilities within their communities. We proudly support more than 1,100 individuals and their families throughout the Greater Capital Area by identifying barriers to access and developing creative, person-centered solutions that foster meaningful community engagement.
Position Overview
The Medical Direct Support Professional (DSP) serves as a community connector, supporting individuals with medical and developmental needs to build meaningful relationships and inclusive community experiences. This role focuses on community mapping, promoting social inclusion, and supporting individuals in pursuing their interests, strengths, and goals-always emphasizing abilities over disabilities.
Qualifications
Education: High School Diploma or GED required
Experience: Experience in human services or working with individuals with disabilities preferred
Requirements:
Reliable transportation
Valid driver's license
Proof of automobile insurance
Compensation & Benefits
$1,000 Sign-On Bonus
$500 paid at hire
$500 paid after 6 months of employment
Bonus available to new hires only; rehires and transfers are not eligible at this time
Paid Training: Employees are paid their regular rate while attending required training
Mileage Reimbursement: Approved mileage driven while on the clock is reimbursed

Less Than Part-Time:
Mondays & Thursdays
7:00 a.m. – 3:00 p.m.
- Saturdays: 7-3pm & 3-11pm
Key Responsibilities
Assist individuals with planned activities and community involvement
Support medical needs including:
G-tubes
Catheters
Medication administration
Oxygen use
Aspiration precautions and other medical supports
Assess pain and discomfort in non-verbal individuals
Develop relationship maps to identify social inclusion opportunities
Support individuals in building and maintaining social networks
Promote self-esteem through active community participation
Provide individualized supports including personal care and skill development
Assist with hygiene and personal care (bathroom use, bathing, dental hygiene, etc.)
Participate in Person-Centered Planning and implementation of goals
Complete required documentation, data collection, and monthly progress notes
Assist with training and orientation of new staff as needed
Distribute medication in accordance with He-M 1201
Report all safety, health, or human rights concerns immediately
Ensure compliance with state regulations
Perform other duties as assigned
