Position Summary

The Medical Records & Referral Coordinator will be responsible for processing all incoming and outgoing medical records as pertains to NeighborHealth Center patients and will assist patients by receiving incoming referrals to NHC and sending referrals to outside facilities and coordinating referral appointments.

Principal Duties and Responsibilities

Medical Records

  • Verify patient demographics in electronic medical record as well as on all printed medical records.
  • Process, route and properly link all incoming electronic faxes from NHC Shared Drive.
  • Upload incoming records to correct patient chart and route to ordering or requesting provider.
  • Link and release orders with related results when appropriate as part of the routing process.
  • Route incoming faxed prescription refill requests and prior authorization requests to Nurse Triage Team.
  • Process all medical records requests by verifying the request is appropriate, verifying the patient status with NHC, and sending appropriate records as requested to requesting entities.
  • Assist patients requesting medical records from NHC by having them complete the appropriate Medical Records Request form, ensuring that all necessary information is obtained.
  • Accepts ownership and responsibility for all actions, tasks, and requests to enhance medical records management and HIPAA compliance.

Referrals

  • Track and document referrals to keep NHC staff aware of referral status in Epic.
  • Ensure complete and accurate patient registration, including demographic and current insurance information.
  • Assemble information concerning patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialists.
  • Contact review organizations and insurance companies to ensure prior approval requirements are met. Present necessary medical information such as history, diagnosis, and prognosis.
  • Ensure that referrals are sent in a timely manner and appropriate documentation is in patient's chart.
  • Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health.

Other

  • Assist with medical assistant or front desk duties as necessary
  • Communicate with patients, providers, and coworkers with a courteous and professional attitude.

Requirements

Required Skills or Abilities

  • Computer literacy in internet use and Windows environment, including Outlook, Word, and Excel.
  • Strong organization skills.
  • Ability to follow and maintain consistent processes and procedures.
  • Ability to cultivate and develop inclusive and equitable working relationships with co-workers and community members.
  • Ability to serve as an advocate for individuals of all ethnicities, genders, ages, and backgrounds.
  • Ability to take initiative and proactively assess processes within referral management.
  • Adequate written and verbal communication skills for communicating coherently and professionally with patients and co-workers.
  • Demonstrates a knowledge of departmental and organizational policies and procedures, and able to embrace and personify the mission of the NHC.

Required Knowledge, Experience, or Licensure/Registration

  • High school diploma or equivalent.
  • At least 6 months of previous medical office experience.
  • Knowledge of HIPAA compliance and requirements.
  • Ability to work onsite Mondays through Fridays during office hours.
  • Ability to read, write, speak, and comprehend English fluently.
  • Up-to-date immunizations.

Preferred Knowledge, Experience, or Licensure/Registration

  • Epic Experience
  • Spanish language fluency

Physical requirements of the Job

Sitting or standing (often for prolonged periods)

Carrying or lifting objects up to 20 pounds

This is a full-time, non-exempt position.
