What You'll Do
Assess patients' psychosocial, financial, and environmental needs
Complete initial and ongoing social work evaluations
Support care planning and discharge planning
Connect patients and families with community resources
Communicate changes and concerns with care team and physicians
Document visits and maintain compliance with care standards
Provide counseling and guidance to patients and caregivers
What You Need
Master's or Doctoral Degree in Social Work (CSWE accredited)
Active Social Worker license
1+ year of healthcare social work experience
Strong communication and interpersonal skills
Ability to travel within the coverage area (50%)
Valid driver's license, auto insurance, and reliable transportation
Ability to lift, push, or pull up to 50-100 lbs
