What You’ll Do
✔ Assess patients’ psychosocial, financial, and environmental needs✔ Complete initial and ongoing social work evaluations✔ Support care planning and discharge planning✔ Connect patients and families with community resources✔ Communicate changes and concerns with care team and physicians✔ Document visits and maintain compliance with care standards✔ Provide counseling and guidance to patients and caregivers
What You Need
✔ Master’s or Doctoral Degree in Social Work (CSWE accredited)✔ Active Social Worker license✔ 1+ year of healthcare social work experience✔ Strong communication and interpersonal skills✔ Ability to travel within the coverage area (50%)✔ Valid driver’s license, auto insurance, and reliable transportation✔ Ability to lift, push, or pull up to 50–100 lbs
